how to get an OPD card at the Philippine Heart Center

When we checked up at the heart center, we applied to the OPD.
(noong nag pacheck up kami sa heart center, nag apply kami sa OPD.)

OPD means outpatient department. It is the one that gives a discount to people who applied here. It depends on your lifestyle.
(Ang ibig sabihin ng OPD ay departamento ng outpatient. Ito ang nagbibigay ng discount sa mga taong nag-apply dito, depende ito sa iyong pamumuhay.)

The requirements for applying to OPD are :
(Ang mga kinakailangan para sa pag apply sa OPD ay)

*the refferal letter from the doctor who last looked at the patient.
(reffferal letter ng doktor na huling tumingin sa pasyente.)

*Social Case Study Report from the Social Welfare Office of the patient's city or town.
(Social Case Study Report galing sa Social Welfare Office ng lungsod o bayan ng pasyente.)

*Latest 1 Month Payslip.
(Pinakahuling 1 Buwang Payslip.)

*1pc 1x1 ID pictures and 1 pc whole body picture of the patient.
(1 pc 1x1 ID pictures at 1 pc whole body picture ng pasyente.)

*Recent SSS/GSIS pension voucher of the retired family member.
(Pinakahuling SSS/GSIS pension voucher ng pensyonadong myembro ng pamilya)

*Member Data Record (MDR) form from philhealth or Certification from HMO if available.
(Member Data Record (MDR) form galing sa philhealth o Sertipikasyong galing sa HMO kung merun.)

*Certification from the place of employment of the patients, spouse, parents (if the patient is a minor o unmarried.
(Sertipikasyong galing sa pinagtratrabahuan ng pasyente, asawa, magulang (kung menor de edad o walang asawa ng pasyente)

*Certificate of Employment Seperation if left/fired.
(Certificate of Employment Seperation kung umalis/natanggal sa trabaho.)


This is an example of an opd card


When you get an Opd card, you will know what category you are and what percentage you will pay, for example C3 25% share, you will only pay 25% like us.
(Kapag nakakuha ka ng OPD card, malalaman mo kung anong category ka at ilang percentage ka at ilang percentage ang babayaran mo, halimbawa C3 25% share nalang ang babayaran mo.)

These are the categories that they will definitely give you after they interview you.
(Ito ang mga kategorya ng tiyak na ibibigay nila sayo pagkatapos nilang makapanayam.)


D: 15% share
C: 20% share
C3: 25% share
C2: 50 % share
B: pay full

(Bago ka maadmit para sa operasyon, ang pasyente at kasama ay nangangailangan ng swab test. hintayin ang result bago ka maadmit.) Qualitative Covid-19 Antigen rapid test price: 1200 The antigen will be done on your companion, the result will be available after 4 hours. (Ang antigen ang gagawin sa kasama mo, ang resulta ay makukuha pagkatapos ng apat na oras.) RT PCR SARS COV2 test price: 4700 PCR will be done to the patient but the price is more expensive than the companion.
